In project part 3 The Flow chart does not show a complete process – the scan post discharge file step does not seem to have any connection arrow where its coming from. Include the steps to from collecting/secure medical records post discharge, confirm receipt of all appropriate records, actions to resolve missing records, steps to complete the scanning process.
