The first step is to understood what is meant by the external environment. This environment includes a number of socio-economical factors that effect the relative health care that a person received. The three factors identified in this question involve regulatory, social and economical. The question asks you to identify one threat and one opportunity for health care within each of these factors.

Regulatory:
There are numerous ways in which state and federal regulations can have a good or bad effect on health care. The one example I found was in reference to Native Americans covered by the IHS (Indian Health Service). When funding to Medicaid and Medicare is cut the IHS feel a greater impact because they rely on these services to make up for underfunding of the agency.
